Biological age is a real research concept and a heavily marketed consumer product, and the gap between the two is wide. In research it refers to specific, validated measures such as epigenetic clocks that predict mortality and disease better than chronological age does. In consumer form it is often a lifestyle questionnaire dressed as a biomarker. Knowing which kind of number you are looking at determines how much weight it deserves.

What the research measures

The best-established approach uses DNA methylation, chemical marks on DNA that change with age in a regular enough way to be read as a clock. Successive generations of these clocks have improved: the earliest were trained to predict chronological age, while later ones were trained directly on mortality and disease risk and predict outcomes substantially better as a result.

Other approaches combine routine clinical measurements, such as albumin, creatinine, glucose, C-reactive protein and white cell counts, into a composite that outperforms chronological age for predicting mortality. These have the practical advantage of using tests people already have, and the disadvantage of being sensitive to acute illness at the time of sampling.

Why consumer results are unreliable

The most serious practical problem with consumer epigenetic tests is test-retest reliability. Studies submitting duplicate samples from the same person have found differences of several years between results, which is larger than most of the differences these tests are marketed to detect. A result that moves by three years when nothing about the person changed cannot detect a two-year improvement from an intervention.

The second problem is that no consumer test has demonstrated that acting on its number improves any outcome. The clocks were validated as predictors in cohorts, not as targets for intervention, and a measure that predicts risk does not automatically become useful when you try to change it.

What actually moves the underlying risk

The interventions with strong evidence for reducing age-related disease and mortality are unglamorous and well established: not smoking, regular physical activity, maintaining muscle mass and cardiorespiratory fitness, adequate sleep, treating hypertension and dyslipidaemia, limiting alcohol, and maintaining social connection. None of these requires a biological age test to identify.

This is the practical reason for scepticism about the category. The measurement is uncertain, the response to intervention is unvalidated, and the actions it would prompt are the same ones already indicated. A test that cannot change what you should do is an expensive way to feel measured.
