Lipase and troponin are two of the most frequently ordered urgent blood tests, and they are often drawn together because upper abdominal pain and chest pain overlap in presentation. They work in opposite ways as diagnostic tools: lipase is interpreted against a multiple of the upper limit of normal, while troponin is interpreted against a percentile cut-off and, more importantly, against how it changes over a few hours.

Lipase and acute pancreatitis

Lipase is an enzyme produced by the pancreas to digest fat. When pancreatic acinar cells are injured, lipase leaks into the bloodstream. The diagnostic threshold in the revised Atlanta classification is a serum lipase at least three times the upper limit of normal, in combination with characteristic abdominal pain; imaging is required only when those two features are not both present.

Lipase rises within three to six hours of onset, peaks around 24 hours and stays elevated for up to two weeks, which gives it a longer diagnostic window than amylase and makes it the preferred test in late presentations. The height of the elevation does not indicate severity: a lipase of twenty times normal does not mean worse pancreatitis than one of five times normal, and severity is assessed clinically and radiologically instead.

Troponin and myocardial injury

Cardiac troponins I and T are structural proteins of heart muscle that enter the circulation when myocytes are damaged. The Fourth Universal Definition of Myocardial Infarction sets the threshold for myocardial injury at a value above the 99th percentile of a healthy reference population, with that percentile defined separately for each assay. There is no universal troponin number, which is why results must be read against the reporting laboratory range.

The critical distinction the definition makes is between injury and infarction. A troponin above the 99th percentile establishes that myocardium has been damaged; it does not establish why. Infarction additionally requires evidence of ischaemia, such as symptoms, ECG changes or imaging findings, together with a rise or fall in the troponin value. A single elevated troponin in isolation is one of the most commonly over-interpreted results in medicine.

Why both are often ordered together

Pain in the epigastrium and lower chest is anatomically ambiguous. Inferior myocardial infarction classically presents with epigastric pain and nausea, and severe pancreatitis can produce chest discomfort and ECG changes. Ordering both tests in an undifferentiated presentation is common practice, not duplication.

The two results also interact. Acute pancreatitis itself can raise troponin without coronary occlusion, through systemic inflammation and haemodynamic stress, and kidney impairment raises both markers by reducing clearance. This is another reason neither value is interpreted alone: the pattern across serial samples, the ECG and the clinical picture carry more weight than any single number.

How high does lipase have to be to diagnose pancreatitis?

At least three times the upper limit of normal, combined with characteristic abdominal pain. The revised Atlanta classification requires two of three criteria: typical pain, lipase or amylase at three times the upper limit, and characteristic imaging findings.

Does a higher lipase mean more severe pancreatitis?

No. The height of the elevation does not track with severity. Severity is judged from clinical assessment, organ dysfunction and imaging, not from how many times the upper limit the enzyme reached.

What troponin level means a heart attack?

There is no single number. The threshold is the 99th percentile of a healthy population for the specific assay used, and infarction additionally requires evidence of ischaemia plus a rise or fall across serial samples. An isolated elevated troponin indicates myocardial injury, not necessarily infarction.

Can troponin be raised without a heart attack?

Frequently. Kidney disease, heart failure, myocarditis, pulmonary embolism, sepsis, arrhythmia and even prolonged strenuous exercise all raise troponin. This is why the pattern across serial samples and the clinical context matter more than a single value.

Why does the emergency department repeat the blood test after a few hours?

Because the change between samples is the diagnostic information. A stable elevation suggests chronic injury, while a clear rise or fall indicates an acute event. Modern high-sensitivity assays allow that second sample as early as one hour after the first in some protocols.
