Identifying an insect bite from its appearance is unreliable, because most bites produce the same thing: a red, itchy papule from a local reaction to injected saliva. What a chart can usefully do is flag the small number of patterns that change management, namely an expanding rash after a tick bite, a lesion that becomes necrotic, a cluster in a line on skin covered by bedding, and any bite followed by breathing difficulty.
Bites that need medical attention
These take priority over identification. In each case the action does not depend on being certain which insect was responsible.
- Difficulty breathing, throat or tongue swelling, widespread hives, dizziness or collapse after any sting: this is anaphylaxis. Use adrenaline if available and call emergency services.
- An expanding red rash, often but not always with central clearing, appearing days to weeks after a tick bite, with or without fever and joint aches. This is the erythema migrans of Lyme disease and needs prompt antibiotic treatment.
- Fever, severe headache or a rash spreading from wrists and ankles after a tick bite in an endemic area.
- A bite that becomes a painful dark or purple lesion that breaks down over days, which can indicate a necrotic spider bite.
- Spreading warmth, redness and tenderness with fever several days after any bite, indicating secondary cellulitis.
- Any bite in someone who is immunosuppressed, or a bite acquired abroad followed by fever.
Typical patterns
The table describes the usual presentation of common bites. Overlap is substantial and the pattern of the bites, where they are, how many, and in what arrangement, is often more informative than any single lesion.
- A line or cluster of three or four bites on skin exposed during sleep is the classic bedbug pattern, sometimes called breakfast, lunch and dinner.
- Most lesions blamed on spiders are not spider bites. Skin infections and other arthropod bites are far more common causes of the same appearance.

Tick removal and what follows
Remove a tick promptly with fine-tipped tweezers, gripping as close to the skin as possible and pulling steadily upward without twisting or crushing the body. Do not apply heat, petroleum jelly or nail polish; these do not make the tick detach and delay removal, which is the thing that matters. Transmission risk for Lyme disease rises substantially with attachment time, so early removal is the single most effective step.
After removal, clean the area and note the date. The relevant window is the following weeks: an expanding rash, fever, joint aches or facial weakness in the three to thirty days after a bite should be reported to a clinician along with the date of the bite. A small red bump at the site immediately after removal is a normal local reaction and not an early rash.
