Reflexology is the practice of applying pressure to specific points on the feet, said to correspond to organs and systems elsewhere in the body. This page shows the zone map as practitioners use it and then sets out what the practice has and has not been shown to do, because the two are frequently conflated. The short version: there is no anatomical connection between these zones and the organs they are mapped to, and reflexology should not be used to diagnose or treat disease.
The zone map as practitioners use it
The map below is the conventional arrangement taught in reflexology, in which the sole is treated as a scaled representation of the body: toes correspond to the head and neck, the ball of the foot to the chest, the arch to the abdominal organs, and the heel to the pelvis and lower back. The medial edge of the foot is mapped to the spine. It is presented here as description, not as anatomy.
- Different reflexology traditions publish different maps, and they disagree with one another on the placement of several organs.
- The disagreement matters: if the zones reflected a real anatomical relationship, independent maps would converge rather than diverge.

What the evidence supports
There is no known anatomical or physiological mechanism connecting discrete points on the sole to specific internal organs. The nervous system does not carry that kind of somatotopic organ map to the foot, and no imaging or physiological study has demonstrated one. Reflexology should not be used as a diagnostic method, and any claim that a tender point on the foot identifies disease in a corresponding organ is not supported.
What foot massage plausibly does is more modest and worth stating on its own terms. Firm, attentive touch is relaxing, and small trials in nursing settings report short-term reductions in anxiety and self-reported pain in contexts such as cancer care, dialysis and post-operative recovery. Those studies are generally small, hard to blind and prone to placebo and attention effects, so they support reflexology as a comfort measure rather than as a treatment.
- Reasonable expectation: a pleasant, relaxing foot massage that may temporarily ease tension or anxiety.
- Not supported: diagnosing conditions, treating organ disease, correcting hormonal problems, or substituting for medical care.
When foot pressure is not safe
Reflexology is generally low risk on healthy feet, but there are specific situations where firm foot pressure is a genuinely bad idea, and they are not obvious to someone booking a session.
- Suspected or confirmed deep vein thrombosis: pressure and massage on the affected leg risk dislodging a clot. Calf pain and swelling need medical assessment first.
- Diabetes with peripheral neuropathy or foot ulceration: reduced sensation means pressure injury can occur without pain being felt.
- Peripheral arterial disease, active foot infection, gout flare, open wounds, recent fracture or recent foot surgery.
- Severe osteoporosis or bone metastases affecting the foot.
- In pregnancy, some practitioners avoid certain points; the practical point is to tell the practitioner you are pregnant and to raise it with your midwife or doctor.
- Most importantly: not as a reason to delay assessment of a new or worsening symptom.

- The zone maps have no demonstrated anatomical basis, and different traditions place the same organs in different locations.
- Reflexology has not been shown to diagnose any medical condition, and a tender point does not indicate disease in a mapped organ.
- Trials reporting relaxation or pain benefits are typically small, short and difficult to blind, so they cannot separate the specific technique from general massage, attention and expectation.
- Any benefit reported is short term and symptomatic. No study supports reflexology as treatment for the underlying cause of any disease.
- The safety exclusions above are absolute rather than cautionary, particularly suspected deep vein thrombosis and neuropathic or ulcerated feet.

Who should avoid reflexology?
Anyone with suspected deep vein thrombosis, diabetic neuropathy or foot ulceration, peripheral arterial disease, active foot infection, an open wound, recent fracture or foot surgery, or bone disease affecting the foot. Pregnancy warrants telling both the practitioner and your maternity clinician.
Why do different reflexology charts disagree?
Because the maps are conventions passed down within different traditions rather than descriptions of anatomy. If they reflected a real physiological relationship, independently derived maps would converge; instead they differ on the placement of several organs.
