A reconstitution calculator answers one arithmetic question: after adding a given volume of diluent to a lyophilised vial, how much liquid contains the amount you intend to draw. That arithmetic is where a specific and well-documented category of harm occurs, because the syringes used are marked in insulin units rather than millilitres, and confusing the two has caused serious overdoses. This page covers the maths and the hazards. It gives no doses and endorses no substance.

Where the errors happen

The dangerous confusion is between units and millilitres. On a U-100 syringe, 100 units is one millilitre, so a person intending to draw 10 units who instead reads the syringe as millilitres, or who applies a dose figure quoted in units for a differently reconstituted vial, can deliver ten times the intended amount. Regulators have issued specific warnings about exactly this pattern with compounded injectables supplied in vials rather than pre-filled pens.

The second common error is the milligram to microgram conversion. Quantities are frequently quoted in micrograms while vial contents are labelled in milligrams, and a factor of 1000 mistake in that direction is not survivable arithmetic to get wrong.

The third is assuming a dose figure transfers between vials. A unit count is meaningless without knowing the concentration it was calculated for, because the same volume from a differently reconstituted vial contains a different amount.

Reconstitution and storage basics

Lyophilised peptides are supplied as a dry powder because they are unstable in solution. Diluent is added slowly down the side of the vial rather than directly onto the powder, and the vial is swirled rather than shaken, because agitation can denature peptides. The powder should dissolve to a clear solution; cloudiness or visible particles indicate a problem and the vial should not be used.

Bacteriostatic water contains benzyl alcohol as a preservative and permits multiple withdrawals over a period; sterile water contains no preservative and is single-use. That distinction matters for contamination risk. Reconstituted peptides are generally refrigerated and have limited stability, and any product that has been frozen and thawed, left at room temperature, or has changed appearance should be discarded.

What this calculator does not tell you

Most peptides sold direct to consumers are marketed as research chemicals not intended for human use, and are outside the regulatory system that assures identity, purity, sterility and potency. Independent testing of such products has repeatedly found contents that differ from the label. A calculator can tell you what volume corresponds to a stated quantity; it cannot tell you that the vial contains what the label claims.

Compounded versions of approved drugs occupy a different but related category. They are not FDA-approved products, are not reviewed for safety, effectiveness or quality before marketing, and the dosing-error warnings referenced above arose specifically from them. Anything intended for human use should come through a prescriber and a regulated pharmacy.

How do I calculate concentration after reconstitution?

Divide the vial contents in milligrams by the millilitres of diluent added. A 5 mg vial reconstituted with 2 mL gives 2.5 mg/mL. The volume to draw is then the intended amount divided by that concentration.

What is the difference between units and millilitres?

On a U-100 insulin syringe, 100 units equals 1 mL, so 10 units is 0.1 mL. Confusing the two is the mechanism behind documented tenfold overdoses, and is the single most important thing to get right.

Can I use a unit figure someone else calculated?

No. A unit count depends entirely on how that specific vial was reconstituted. The same number of units from a vial diluted differently contains a different amount. Always recalculate for your own vial and diluent volume.

What is the difference between bacteriostatic and sterile water?

Bacteriostatic water contains benzyl alcohol as a preservative and allows multiple withdrawals over a period. Sterile water has no preservative and is single-use. They are not interchangeable, and benzyl alcohol is not appropriate for neonates.
